What is person-centred support?

What we mean by person-centred support

Everyone needs support from time to time, this is also true for people with disabilities. How this support looks changes over time depending on our abilities, needs and circumstances. We create personalised plans for every person we support so they feel empowered to live the life they want on their terms. These plans involve the person we support, the people who are important to them and their support teams to ensure we create a whole picture of someone’s needs. Person-centred support enables people to meet their basic needs as well as experience exceptional moments. We believe person-centred support is the best way to create full lives.
